Benefits overview
Annual Open Enrollment in your opportunity to add/drop dependents, change coverage, drop coverage or elect additional coverage. However, during your employment, you may update or make changes to your benefit(s) when you experience a qualifying life event or employment change. When you experience a qualified life event, you have 30 days of such event to notify the HR Department as well as supporting documentation. If you miss this deadline, you will need to wait until the next annual open enrollment period to make changes to your benefits. Based on IRS rules, you can generally make changes during the year only if you have a qualifying change in your family or employment status.

- Marriage or divorce
- Birth or adoption
- Change in employment status for you or your dependent, when the change affects eligibility
- Loss of eligibility of other health care coverage/program
- Loss of eligibility for your child because of marriage, exceeding the plan’s age eligibility limit or no longer being disabled
- Death of your spouse or your child
- Change in day care providers, the hours for which care is needed, or the cost of day care
- Change in other medical or dental coverage as a result of the annual enrollment for the employer of your spouse or eligible child occurring at a different time than Prevea
- When you have a qualifying life event change, you have within 30 days of such an event to notify Prevea’s Benefits Team.
You can enroll eligible family members for medical, dental, Optional Life Insurance and AD&D coverage. Eligible dependents include:
- Your spouse to whom you are legally married. Does not include common law marriages.
- Dependent children up to age 26
- Unmarried dependent child(ren) of any age who has a physical or mental disability and is incapable of self-sustaining employment, as long as the disability begins before the child reaches age 26; you must provide proof of the child’s disability.
All regular scheduled full-time (who work a minimum of 30 hours per week) and part-time employees (who work a minimum of 20 hours per week) are eligible for benefits. Casual and PRN employees are eligible to contribute to the Retirement plan. Seasonal, flex, interns and temporary colleagues are not eligible for benefits.
