Are you new to Prevea Health and looking for a primary care provider or specialist? Do you have questions on how to transition your care to Prevea Health?

Let one of our patient advocates help! We are here to assist you with the following:
  • Select a new primary care provider

  • Establish and navigate the MyPrevea online patient portal

  • Review preventative services needed such as a physical, mammogram or colonoscopy

  • Assist with scheduling a primary care provider or specialist visit

  • Transition medical records from outside health care facilities
