Prevea’s DOT physical exams follow strict guidelines mandated by the Federal Motor Carrier Safety Administration (FMCSA), ensuring that commercial drivers and other safety-sensitive employees are in good health to work safely. Our medical examiners are certified by the FMCSA and our exam meets DOT and FMSCA requirements. Our goal is to help you through this process to help you qualify for your FMCSA card and ensure your safety.

Federal and state regulations require all Commercial Driver License (CDL) holders to certify their tier of operation — the type of operation in which they are engaged. The driver’s Tier of Operation determines any medical requirements.

Prevea also provides DOT physical exams for workers other than drivers. For example: school bus drivers, tractor/trailer technicians, highway maintenance technicians, engineering equipment operators, terminal managers, delivery drivers, etc.

Employment physical exams and screenings

Commercial driver’s license holders and applicants will undergo a medicolegal examination which is conducted by a certified medical examiner (CME) who is registered with the Federal Motor Carrier Safety Administration (FMCSA) and linked with its National Registry.

The clinic will provide drivers with medical forms at the time of the visit on which they will document their past and current health status, medical and surgical history, medications and other treatments such as positive airway pressure for obstructive sleep apnea.
A vision screening, hearing test, blood pressure, height, weight and urinalysis will be completed. An overall physical examination will be performed, including:
- General appearance
- Skin
- Eyes
- Ears
- Mouth/throat
- Cardiovascular
- Lungs/chest
- Abdomen
- Genitor-urinary system including hernias
- Back/spine
- Extremities/joints
- Neurological system including reflexes
- Gait
- Vascular system
Upon completion of the exam, a qualified driver will be issued a Medical Examiner’s Certificate (Fed Med Card):
- Prevea will electronically enter into the National Registry.
- Drivers are responsible for uploading their Fed Med Card with the state.
- For intrastate drivers, there is a section on the Fed Med Card indicating the driver is qualified with any applicable state variances, which will only be valid for intrastate operations.

Persons with sleep apnea who are treated with positive airway pressure must provide data:
- Printout indicating the machine is used at least 4 hours per night, at least 70% of the time during a one year period
- The AHI number which indicates how well treatment is working
The physician or provider (Certified Medical Examiner, CME) will review available electronic medical records. If the information is not complete or if records are not available, the examiner will instruct the driver of what is needed.
Persons with insulin treated diabetes (ITD):
- Medical qualification is determined by the CME; a federal waiver is no longer required. The driver must provide to the CME at the time of the physical an Insulin-Treated Diabetes Mellitus Assessment Form completed by the treating health care provider to attest that the driver has a stable insulin regimen and properly controlled diabetes. The form must be completed by the treating health care provider no more than 45 days prior to examination by a CME. A Medical Examiner’s Certificate cannot be issued to the driver without this form.
- A driver with ITD who drives intrastate (and does not need a Fed Med card) and who is not “grandfathered in,” must apply to the state for a diabetes waiver. The state, not the CME, will determine whether the driver is qualified to operate a commercial motor vehicle. The state requires the driver to submit to the state two separate Medical Examination Reports (MV3644) completed by two different health care providers; and examinations must be conducted within 90 days of its receipt at the DMV. The CME can be one of the health care providers who can help drivers through the process.
