Prevea Cancer Connect
Cancer nurse navigation. An advocate for your cancer care.
A cancer diagnosis can be overwhelming. It’s natural to have questions and not know where to start. That’s where Prevea’s nurse navigators come in. You’re not alone on your cancer journey.
Our purpose
- To create a trusted connection with our patients,
- To understand your needs,
- To personalize your journey,
- To make the connections you need,
- To ensure you are not left seeking instead of healing your mind, body and spirit.
Your Prevea nurse navigators
From your diagnosis through treatment and follow-up, a nurse navigator is available to provide personalized care for you as a dedicated member of your cancer team. The navigator will clarify and reinforce information about your diagnosis and treatment and identify and work through barriers to care. We will help you locate services and support systems, provide patient resources and empower you to become an informed participant in your cancer care.

When is the right time to connect with a navigator?
If you are worried you have cancer or if you have been recently diagnosed with cancer, a conversation with a navigator will help answer any questions that you may have.
To meet with a navigator, call (833) 870-1056 or (920) 272-1224 or schedule a conversation online.
We encourage you to contact us at any time with any questions or concerns so we can work through them together. Patient navigation services are available as often as needed by our patients.
