Your company has partnered with Prevea Health for various health care services available to you for a minimal fee.

Appointments for urgent care, primary care (family medicine, internal medicine and pediatrics) and physical and occupational therapy are available at all Prevea Health locations where those services are offered.


HSHS St. Clare Memorial Hospital Prevea Health Centers are EXCLUDED, including Gillet, Lena, Mountain, Oconto Falls and Suring.

The Partnered Health access card must be presented at time of check-in; otherwise, the service will be billed to your personal health insurance. Prevea360 health insurance members do not have a Partnered Health card; please show your Prevea360 member insurance card.

The following labs are available at no additional cost to you as the patient with a Prevea Partnered Health physician or provider order. Labs not listed or ordered by a non-Prevea Partnered Health physician or provider will be billed to your personal health insurance.
